Requirements:

Reserve one or more training slots:
Before July 1, 2010
Deposit for each training slot:
Initial slot – $1,000
Recurrent slot – $500
(Deposits are non-refundable)
Scheduling:
Training must be completed by December 31, 2010.
- Training dates need not be confirmed at the
  time of deposit.
- Training dates require 21 day prior notice.
Total costs guaranteed in writing at time of deposit.
Balance due on first day of training.
